Chicago Family Case Management
Provides case management and outreach services
to pregnant women and infants, 0-1 years of age who
receive coverage under the state's medicaid program.
Assist individuals in gaining access to well child
services, early prenatal care and linkage to needed
social services.
Health Works:
Provide case management services to wards of the
state 0-5 years of age.
Men's Project:
The goal is to reduce the rate of HIV/AIDS infection
among males who have unprotected sex with partners who engage in high risk activities. HIV/AIDS education, counseling, case management and referral
services are provided.
Women's Project:
HIV/AIDS and STD education, one-on-one counseling, safe sex, risk education, alcohlo and other
drugs information, referral services, peer education, pre & post test counseling.
